Registration Forms are available for completion Online. There are 2 methods of payment for the symposium each with corresponding forms. You can pay by check by mailing the completed form and a check. Alternatively you can pay by PayPal. You do not need a PayPal account to utilize this method of payment. A 3% “Tax” will be automatically added to cover the cost of the PayPal transaction. Once you have completed the PayPal registration form and reviewed it you will be re-directed to PayPal for payment.
